Medical Examination Fee: The per-person fee for a physical exam is $150.00 for applicants 15 years and older and $120.00 for applicants under 15 years of age. The fee includes chest x-ray, serologic test for syphilis and HIV, physical exam and documentation. Prices of the laboratory tests and medical examinations are subject to change.
