There are many government forms used during the adjustment of status process. Some of these forms may not be required in your circumstance. Due to the complexity of this process, you are encouraged to seek assistance (We can help) so you don’t need to worry about navigating the seemingly endless pile of paperwork.

Purpose

I-485 Application to Register Permanent Residence or to Adjust Status with two attached photos (see below).
I-131 Application for Travel Document (Advance Parole): Needed if alien wishes to travel outside the US during the I-485 process.  Can be filed concurrently with the I-485 or afterwards without a fee.
I-765 Application for Employment Authorization (Work Permit).  Only if applicant desires work authorization while I-485 is pending.  Can be filed with the I-485 or afterwards without a fee.
G-325A Biographic Information Sheet
I-693 Medical Examination of Aliens Seeking Adjustment of Status:  With accompanying I-692 Vaccination record. Both records are submitted in the sealed envelope.
I-94 Nonimmigrant Arrival/Departure Record:  Needed to verify lawful admission into the US and maintenance of status.  Copies of all periods of stay in the US and all pages from passport must be included.
I-864 Affidavit of Support
G-1145 E-Notification of Application/Petition Acceptance
I-130 Petition for Alien Relative:  If adjustment of status is family based or being filed concurrently with the I-485
