Appendix of Adjustment of Status Forms

Form Purpose
I-485 Application to Register Permanent Residence or to Adjust Status with two attached photos (Form I-485, Explained here).
I-131 Application for Travel Document (Advance Parole): Needed if alien wishes to travel outside the US during the I-485 process. Can be filed concurrently with the I-485 or afterwards with a separate filling fee.
I-765 Application for Employment Authorization (Work Permit). Only if applicant desires work authorization while I-485 is pending. Can be filed with the I-485 or afterwards with a separate filling fee.
G-325A Biographic Information Sheet
I-693 Medical Examination of Aliens Seeking Adjustment of Status: With accompanying I-692 Vaccination record. Both records are submitted in the sealed envelope.
I-94 Nonimmigrant Arrival/Departure Record: Needed to verify lawful admission into the US and maintenance of status. Copies of all periods of stay in the US and all pages from passport must be included.
I-864 Affidavit of Support
G-1145 E-Notification of Application/Petition Acceptance
I-130 Petition for Alien Relative: If adjustment of status is family based or being filed concurrently with the I-485
I-944 Declaration of Self-Sufficiency
