The USCIS has updated to a new version of form I-693, Report of Medical Examination and Vaccination Record. However, up until the time of this post, they are still accepting the old version of the form. Starting September 23, 2019, the USCIS will only accept the 07/15/19 edition. Until then, you can use the old version of the I-693 which is the 10/19/17 edition. You can find the edition date at the bottom of the page on the form and instructions.

If you are applying for Adjustment of Status to become a lawful permanent resident, you will use this form to establish that you are not inadmissible to the United States on public health grounds.

As a requirement, this medical exam must be no more than 60 days old when the associated petition is received by the USCIS. Because this medical exam record is sealed, RapidVisa has no way to confirm your form's edition date and therefore it is the responsibility of the applicant to make sure that the edition date is current, otherwise, your entire petition will get rejected by the USCIS or you will receive a request for evidence requesting the new form.
